Its objective is to analyze the effectiveness and efficiency of care trajectories (TA) in four high-prevalence and high-impact chronic health problems: type 2 diabetes mellitus, atrial fibrillation, breast cancer, and ischemic stroke. As a common denominator, CONCEPT shares the innovative perspective of focusing its analyses on TA as a key determinant of care adequacy, treatment adherence, health outcomes, and their economic consequences. The primary objective of CONCEPT-COSTS is to complement the results produced by each clinical cohort of CONCEPT with a broad economic analysis based on real-world data (RWD), including incurred costs, avoidable costs, and the evaluation of the efficiency of identified TAs. These results will inform clinical and management decisions regarding TAs that should be promoted or avoided.
