Objectives and Methodology
Main Objective: Evaluate the longitudinal relationship between person-centered care for chronic diseases (PCC/CD) (and its various sub-dimensions) as perceived by the chronic patient in routine primary care practice, and the following patient outcome variables: 1) health behaviors; 2) health-related quality of life; 3) resource utilization.
Secondary Objectives: Evaluate the effects of the professional’s perception of PCC/CD on the outcome variables, as well as its concordance with the patient’s perspective; assess predictors of the PCC/CD as perceived by the patient.
Method:
- Prospective Observational Study: A cohort study (18 months of follow-up) involving primary care professionals and patients with chronic diseases. Dependent variable: PCC/CD (PACIC and SDM-9-Q questionnaires for patients and professionals, and P-CAT for professionals). Outcome variables: health-related quality of life (SF-36, EQoL), health behaviors (diet, physical activity, tobacco and alcohol use, medication adherence), and resource utilization.
- Qualitative Study: Four discussion groups with patients and professionals aiming to: a) deepen the understanding of the gaps identified in the quantitative study; b) create a map of the chronic patient’s journey from the perspective of PCC/CD.
